To prove your diagnosis you need medical records, and they are often spread across different doctors and clinics going back many years. Your claim needs documents that prove the diagnosis and the date it was made, between March 8, 2015 and March 8, 2019. Here is what to ask for:
Primary lung or throat cancer
A pathology report that confirms the diagnosis (primary lung cancer, or a primary squamous-cell cancer of the larynx, oropharynx, or hypopharynx).
Emphysema or COPD
A spirometry (breathing) test report from those years showing severe impairment, at GOLD Grade III or IV.
If you cannot get those
A copy of the medical file, a completed Physician Form, or a letter from the doctor with a supporting report such as a CT scan, MRI, or biopsy.
What it costs
You will see the full cost before you confirm. There are two parts:
Medchart's fee for each request.
Fees charged by the clinic or hospital, set by them and your responsibility. Often around $30 per request plus a small per-page charge.

Requesting records for someone who has passed away
Many people file a claim for a family member who has passed away. You can request their records here. To file the claim itself, you will also need to show that you are legally allowed to act for their estate.
